What’s my estimated deductible/service fee and repair eligibility?
Estimates are based on the chosen device and UScellular Device Protection Plan. Check your wireless bill or online account at uscellular.com to identify your device protection program
Upload documents
We may ask for documentation to help keep your account safe. If we ask you for this when you start a claim, upload your documents online. We’ll let you know once we review them.

If you have a Device Protection+ Ultimate plan, you might be eligible for same-day delivery and set up if you complete your claim before 4pm local time. Eligibility is determined during the claim process.
Shipping cost is included for all replacement devices.

Will my plan cover a repair or replacement?
1. Claims fulfilled with a replacement device and approved by 12 a.m. CT Mon-Sat (10:00 a.m. Sun) will be shipped and, in most cases, delivered the next day. Replacement devices may be new or refurbished of the same or a like kind and quality model. Phone color, brand, model and features may be different. Compatibility of accessories is not guaranteed. For DP+ Ultimate customers only, claims filed and approved by 4 p.m. (local time) may be eligible to receive a replacement device as soon as the same day. Same day delivery is available in select locations for certain devices, which are subject to change at any time. Availability of delivery is subject to location, replacement and technician availability.
